Court Monitor Application Procedure

Before filling out this application, a certified court monitor applicant must go through initial training provided by a designated court monitor trainer. Once initial training has been completed and the designated trainer believes the trainee is ready for temporary certification, a purchase order for $105.00 from the court or government entity must be sent to us at ccr@ccrboard.com. The purchase order should indicate the name of the applicant. An invoice will be sent to the applicant automatically after the application below is completed and submitted. We will also send the invoice to the email address of the financial department that is provided by the applicant in the application.Trainers and supervisors, please make sure the applicant has the financial department's email address for the invoice.
